A2:Please submit it to the Office of Environmental Protection, Occupational Safety and Hygiene, located on the 2nd floor of Thin Film Technologies Building II.

 

Q3:Can I submit a photocopy of the physical examination report?

 

A3:Yes, you can submit a photocopy, but you need to bring both the original and the photocopy of the physical examination report to the nurse at the Office of Environmental Protection, Occupational Safety and Hygiene for verification. Once the report has ben verified, you can retrieve the original copy.
